The ‘Peer Education for End of Life Project’ began in 2007 and was undertaken by a group of older adults with potentially life-shortening conditions, carers and an academic researcher. This collaboration led to the development of a personal portfolio template and a series of 8 public workshops to raise public awareness around end of life issues and to encourage advance planning at an individual level.

The portfolio template is designed to compliment care delivery from existing care providers and empower individuals to record and store information to support their future care needs.
